Current Work

I'm a department administrator for continuum, complex and community care with Northwest Permanente where I partner with Dr. Briar Ertz-Berger and many other clinical leaders in Permanente and health plan to collaboratively support a number of care delivery departments and initiatives such as Continuing Care Services, Primary care at Home, Palliative Care, Complex Care Medical Home, Social, Economic and Behavioral Health (SEBN) Medical-Legal Partnership and intensive case management. I have a joint appointment with the Kaiser Permanente Center or Health research as a clinical investigator, focusing on interventions that address socio-economic barriers to care and reduce health disparities.

I also support Kaiser Permanente on a national initiative to roll out an enterprise-shared social service resource locater. This work is part of a broader total health strategy and community health initiative to address social, economic and behavioral needs of our members and communities we serve. This work is sponsored by Greg Adams, executive vice president and group president and Dr. Bechara Choucair, senior vice president for community health and chief community health officer. I am the operational lead, supporting the clinical and care delivery integration and implementation, spread and scale of this tool across the KP Program.
